- Valid for selected SURBEX promo pack (50’s/60’s x 2) or single pack (30’s/50’s/60’s) in a SINGLE receipt/ transaction. No combined receipts and/or no split transaction are allowed.
- Receipt/ proof of purchase shall be uploaded into Abbott LifePlus (ALP) account or email to info@abbottlifeplus.com before 30 Sep 2022, 11.59pm.
- Receipt/ proof of purchase received after the stipulated time and(or) failed to abide to ALP Receipt Submission Guideline https://www.abbottlifeplus.com/files/Receipt-Submission-Guideline.pdf will be disqualified and ineligible for the redemption.

- Member who is eligible for RM5 Touch ‘n Go eWallet Reload PIN will be receiving the Reload PIN via SMS notification within 14 calendar days after campaign ended.
