- Valid for receipt with purchased date 22 Apr 2022 onwards from Caring Pharmacy (Caring e-store caring2u or in-store). Not applicable for Lazada and Shopee Caring e-store or other online platform.
- Receipt/proof of purchase shall be uploaded into Abbott LifePlus (ALP) account or email to info@abbottlifeplus.com before 31 May 2022, 11.59pm.
- Receipt/proof of purchase received after the stipulated time and(or) failed to abide to ALP receipt Submission Guideline https://www.abbottlifeplus.com/files/Receipt-Submission-Guideline.pdf will be disqualified and ineligible for the redemption.
- Valid for selected SURBEX promo pack (50’s/60’s x 2) or single pack (30’s/50’s/60’s) in a SINGLE receipt/transaction. No combined receipts and/or no split transaction are allowed.
- Limited to 50 redemptions on a first come-first served basis. Maximum 1 (one) redemption per member.
- Member who is eligible for the redemption will be receiving SMS notification on successful redemption within 14 calendar days after receipt approved.
- Redemption item will be couriered to the eligible member’s delivery address stated in ALP account within 4 weeks after campaign ended.
