1. Valid for Surbex promo pack (50’s/60’s x 2) or single pack (10’s/30’s/50’s/60’s) in a SINGLE receipt/ transaction. No combined receipts and/or no split transaction are allowed.
2. Receipt/ proof of purchase shall be uploaded into Abbott LifePlus (ALP) account or email to info@abbottlifeplus.com before 28 Feb 2023, 11.59pm.
3. Receipt/ proof of purchase received after the stipulated time and(or) failed to abide to ALP Receipt Submission Guideline https://www.abbottlifeplus.com/files/Receipt-Submission-Guideline.pdf will be disqualified and ineligible for the redemption.
4. Maximum 1 (one) 188/288 ALP points reward for 1 (one) member. Limited to 1000 redemptions on a first come-first served basis.
5. Member who is eligible for 188/288 ALP points reward will be receiving SMS notification within 14 working days after receipt/ proof of purchase approved. 188/288 ALP points will be credited into ALP account in 1st week of March.
6. Member who is eligible for Lucky 8 special redemption will be receiving phone call/SMS notification within 14 working days after campaign ended. Special redemption item will be based on random selection (1x Tupperware Supersonic Chopper Set; 2x Perysmith PS1530 Air Fryer; 5x Bear Electric Lunch Box BLB-GP17L) and member is required to complete the redemption with 8 ALP points. Redemption item will be couriered to the delivery address as per stated in ALP account within 4-6 weeks from date of notification.
